USC Clubs Harassment Discrimination and Violence Prevention Reporting Form

The USC Clubs program behaviour management system functions under a complaints-driven model. Any Individual USC Club Member who is the victim of violence or harassment while participating in USC Club activities or who witnesses violence while participating in USC Club activities may file a complaint against, or raise an issue involving, a USC Club member, or USC Club leader, where it is perceived that the individual(s) or group(s) acted in a manner not in compliance with the USC Clubs Discrimination Harassment and Violence Policy, and/or the spirit and goals of the USC Clubs Program.

Within this form, "Complainant" refers to any individual who is a member of a USC Club who deems he or she has been the target of discrimination or harassment. “Respondent” refers to the person who is alleged to have engaged in the discrimination or harassment.

All complaint submissions will be received and investigated by the Senior Manager, People and Development. This office will be responsible for investigating the complaint or appointing appropriate designates to investigate on their behalf in a confidential manner.

Please note: Upon receipt of a report or complaint, the Senior Manager, People and Development will respond to discuss the complaint, the provisions of and options available under this Complaints Procedure within 5 business days.
